Podcast Show Guest Form

By submitting this form, I hereby give permission to BertsParty.com, its affiliates, and agents (collectively, “the Podcast”) to use my images, videos, voice recordings, and any other media captured or recorded during my participation in the BertsParty.com Podcast show. This consent includes, but is not limited to, the right to edit, use, and reuse said products for any and all purposes, such as marketing, promotion, and public relations, in any and all media now known or hereafter devised, worldwide, without limitation or compensation.

I acknowledge and agree that all photos, videos, audio recordings, and any other media produced are the property of the Podcast, and I waive any rights to inspect or approve the finished product, including written copy, wherein my likeness or my voice appears.

Additionally, I waive any right to royalties or other compensation arising from or related to the use of the media. I also understand that this material may be used in diverse settings within an unrestricted geographic area.

Photographic, audio, or video recordings may be used for the following purposes:

Publication on the Podcast’s website and social media pages
Promotion of the Podcast in any media format
Public relations materials related to the Podcast
By agreeing to this release, I am declaring that I am at least 18 years of age, have read and understood the above release, and am competent to execute it. If the participant is under the age of 18, the parent or legal guardian must read and sign this release on their behalf.

I understand that this consent is voluntary and I am free to withdraw my consent at any time. To withdraw my consent, I understand that I must submit my request in writing to the contact details provided by the Podcast.
